Key Responsibilities:
Collaborate with the Multidisciplinary Team: Work closely with mental health professionals, healthcare staff, and external agencies to ensure comprehensive support for service users.
Assessment of Need: Undertake assessments to establish eligibility for services under Fairer Access to Care Services (FACS) and the Care Programme Approach (CPA) for adults with mental health problems, as well as their carers and families.
Provision of Social Work Services: Deliver high-quality social work services to adults with mental health problems, supporting them and their carers through challenging circumstances.
Care Plan Management: Manage, monitor, and review care plans in collaboration with service users, carers, and other professionals. This includes liaising with workers and agencies to ensure the effective delivery of care, including overseeing those in residential accommodation or other purchased placements.
Care Coordination: Act as a Care Coordinator under the Care Programme Approach (CPA), ensuring a person-centred and holistic approach to care planning and implementation.
Financial Accountability: Work within the Council’s Financial Regulations when coordinating care packages and services, ensuring cost-effective solutions are provided without compromising the quality of care.
Resource Awareness: Stay informed about available resources and services within the mental health field to offer the best possible support to service users and their families.
Information Management: Utilize information management systems effectively, ensuring accurate recording and retrieval of data related to service users’ care and assessments.
Record-Keeping Compliance: Adhere to local recording policies and ensure all required documentation is completed in line with the Directorate’s standards.
Supervision and Professional Development: Be open to management, professional supervision, and regular assessments against agreed standards and targets to ensure continuous personal and professional development.
Duty Rotas: Participate in duty rota systems as part of the multidisciplinary team, providing necessary cover and support as required.
Safeguarding Adults: Undertake safeguarding adult enquiries, ensuring the safety and welfare of vulnerable individuals under your care.
Compliance with Policies: Carry out all duties in accordance with the professional code of conduct, Council policies, and Mental Health Trust guidelines, ensuring best practice at all times.
Other Duties: Perform any additional duties appropriate to the role as directed by your line manager or senior management, ensuring flexibility and responsiveness to service needs.
Legislation Awareness: Stay up-to-date with current legislation, government policies, and guidance concerning mental health and safeguarding vulnerable individuals.
Essential Requirements:
Degree/Masters in Social Work and SWE registration.
2+ years of post-qualification experience in mental health or a related field.
Strong knowledge of UK social care legislation, particularly mental health legislation, and practice.
Right to work in the UK.
